Job summary

Beth Israel Lahey Health at Home is seeking a dedicated registered nurse to deliver high-quality home health care. You will perform initial patient assessments, plan and adjust care plans, administer prescribed medications, and mentor home health aides on a regular basis.

Requirements include an active NH RN license, BLS certification, and 1+ year of direct care nursing experience. OASIS/EPIC experience is preferred, with a strong emphasis on HIPAA confidentiality and timely documentation.

Qualifications

• Active New Hampshire RN license.
• Active American Heart Association BLS certification.
• At least 1 year of direct care nursing experience.
• OASIS and EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiates the plan of care and makes revisions as patient needs change.
• Administers medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Maintain confidentiality per HIPAA guidelines.
• Demonstrate proficiency in OASIS completion.
• Complete documentation through EPIC per agency timelines.

• 12 weeks of hands‑on hospice training in an orientation setting
• Designated preceptors
• On going tailor‑made mentorship based on individual needs

Job Description:

What You’ll Do:

• Complete and document an initial assessment of patient and family to determine home care needs;
• Initiates the plan of care and makes necessary revisions as patient status and needs change;
• Administers medications and treatments as prescribed by the physician;
• Instruct, supervise, and evaluate the home health aide care provided every two (2) weeks;
• Identify discharge planning needs as part of the care plan development;
• Practice confidentiality principles set by the agency and Federal HIPAA guidelines;
• Demonstrate proficiency in OASIS completion;
• Complete documentation through EPIC in accordance with agency timeliness policies.

What You’ll Need:

• An active valid New Hampshire RN license;
• An active American Health Association BLS certification;
• Associate’s Degree or BSN (preferred) from an accredited school of nursing;
• At least 1 year of direct care nursing experience, preferably the area of public health or home care nursing;
• OASIS and/or EPIC experience is preferred.
