Job summary

Enhabit Home Health & Hospice in Texas is seeking a licensed professional to administer skilled care to patients in the home setting. You will teach patients and families, work with the health care team, and follow physician orders under the established plan of care (POC).

Requirements include graduation from an approved school of clinical education, active licensure in Texas, at least one year of licensed experience, and basic technology skills including use of a mobile device.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
