Job summary

Pathways Healthcare is a leading home health and hospice organization delivering patient-centered care. We are seeking a Registered Nurse - Home Health to serve Devens, Shirley, and surrounding areas with a Monday-Friday schedule.

Ideal candidates have 2+ years of nursing experience, preferably geriatrics, and a degree in Nursing. Proficiency with EMR, solid bedside manner, knowledge of healthcare laws, and a valid BLS CPR certification are required.

Qualifications

2+ years' nursing experience, preferably in geriatrics.
New nurses, feel free to apply! We have a great training program.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of health care laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

Assesses and evaluates patient status by: writing and initiating plan of care.
Regularly re-evaluating patient and family/caregiver needs.
Participating in revising the plan of care as necessary.
Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
Completes, maintains and submits accurate clinical notes and documents changes.
Communicates with the physician regarding patient needs and reports changes.
Provides and maintains a safe patient environment.
Attends interdisciplinary group meetings.
Participates in on-call duties.
Ensures equipment and services are available and delegates tasks when required.
Pursues ongoing professional development and continuing education.
