Job summary

Enhabit Home Health & Hospice Careers is seeking licensed professionals to administer skilled home health care, teaching patients and families as outlined by the physician and plan of care. Applicants must hold an active state license and have at least one year of experience.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed to practice in the state where they currently work.
• Minimum of one year experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients per physician orders and the plan of care.
• Teach the patient, family, and care team as required by the care plan.
• Perform services in accordance with the physician’s orders and POC.
