Responsibilities

• Initiates, develops, implements and revises the plan of care in collaboration with the physician.
• Supervises home health aides and licensed nurses, instructs and assigns tasks per regulations.
• Provides required supervisory visits.
• Performs comprehensive assessments and medication reconciliation timely.
• Documents visits in accordance with policy and payer requirements.
• Communicates with physicians, patients and families to ensure quality care.
• Participates in on-call and weekend rotations as needed.
