Resource 1 Homecare Staffing in Clinton, MD is seeking a Home Health Registered Nurse (RN) for a full-time, field-based role. You will travel to patient homes to develop individualized care plans and promote optimized outcomes, coordinating with physicians and the home health team.

The position emphasizes patient safety, quality care, and adherence to home health protocols. A valid RN license in the area and CPR certification are required, with 1–2 years of recent RN experience in relevant

Formación

• RN license required (District of Columbia, Maryland or Virginia or as location allows)
• 1–2 years RN experience within last 5 years (ICU/CCU/Med-Surg/Oncology/Gerontology)
• CPR certification required
• IV therapy skills expected
• Strong communication and organizational abilities
• Basic computer skills preferred

Responsabilidades

• Develops and documents an individualized plan of care with specific interventions and measurable patient outcomes
• Evaluates progress toward goals and revises the plan of care as needed
• Establishes, documents, and implements teaching plans for patients and caregivers
• Performs nursing procedures per homecare protocols and uses standard homecare supplies and equipment
• Participates in regularly scheduled case conferences with the care team
• Includes patient and family in mutual goal setting and care plan revisions
• Maintains clinical records, collects data, and prepares activity reports
• Communicates with physician, home health team, and other care team members
