UnitedHealth Group is seeking a Registered Nurse for home health in Illinois. You will provide and direct nursing care to patients in their homes, coordinating the care plan with physicians and the health team to ensure continuity of care.

Responsibilities include developing and revising plans of care, supervising home health aides, documenting visits, performing OASIS assessments, and upholding quality and compliance while delivering compassionate, patient-centered care.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Responsabilités

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
• Provides required supervisory visits
• Provides high-quality clinical services within scope of practice and infection control standards, coordinating with the care team
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
• Documents patient visits per policy and payer requirements, and syncs timely per LHC policy
• Makes nursing evaluation visits, ensures eligibility and medical necessity, and communicates with physicians and staff
• Participates in on-call and weekend rotation as needed
