CenterWell Home Health seeks a Home Health Registered Nurse to provide one-on-one care in patients' homes, reporting to the Clinical Manager. You will manage care plans with physicians and clinicians to help patients regain independence and return to the life they love.

You will administer medications, perform wound care, monitor conditions, educate families, and coordinate with the care team while overseeing Home Health Aides and ensuring proper documentation.

Qualificações

• Med/Surg, ICU, ER, or acute care experience.
• Current unrestricted RN licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Experience collaborating with a team of healthcare professionals.

Responsabilidades

• Provide skilled nursing care in patients' homes.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records and coordinate with other healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
