Job summary

CenterWell Home Health is seeking a compassionate Home Health Registered Nurse to provide one-on-one skilled nursing in patients' homes. You will develop and implement individualized care plans, administer medications, assess patients, educate families, and coordinate with physicians and a Clinical Manager.

You will join a multidisciplinary team, supervise Home Health Aides, and ensure thorough documentation while adhering to TB and driver-safety requirements.

Qualifications

• RN license in good standing required.
• CPR certification required.
• Experience collaborating with healthcare professionals is essential.
• Home health or hospital-based experience preferred.

Responsibilities

• Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes to physician and Clinical Manager.
• Educate patients and families on disease management, medications, and treatments.
• Maintain accurate records and coordinate with other healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
