Qualifications

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).
• Education: Graduation from an accredited nursing program required.
• Licensure: Current RN license in the state of Missouri. Must also have a valid driver’s license and proof of auto insurance.
• Certifications: BLS Certification required through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in the home setting including case management and total care for multiple patients with varying acuity.
• Perform admissions, post-hospitalization assessments, recertifications, discharges, and patient teaching.
• Administer IV therapy, dressing changes, and site care according to policy and procedure.
• Document appropriately using the Epic system and ensure timely updates.
