Qualifications

• Strong clinical judgment and ability to prioritize a full caseload.

Responsibilities

• Manage a caseload of home health patients with visits and care plan updates.
• Perform admissions, recertifications, and discharges with OASIS documentation.
• Coordinate with physicians, therapists, and the interdisciplinary team.
• Educate patients and caregivers on disease management and home safety.
• Respond to changes in patient status and escalate when needed.
• Maintain documentation meeting Medicare, state, and agency standards.
