Job summary

One Home Health LLC is seeking an experienced Home Health Registered Nurse (RN) to join our Elgin, IL team. This role requires managing patient caseloads, performing skilled visits, and updating care plans in patient homes.

You will perform admissions, recertifications, and discharges with precise OASIS documentation and coordinate with an interdisciplinary team to address patient condition changes. Independent practice and reliable transportation are essential.

Qualifications

• A minimum of 1–2 years of RN experience, with home health experience required.
• Active, unencumbered IL RN license.
• Working knowledge of OASIS documentation and Medicare home health guidelines.
• Comfortable working independently in patients' homes across service area.
• Reliable transportation and a valid driver's license.
• Strong clinical judgment and ability to prioritize a full caseload without close supervision.

Responsibilities

• Manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in the field
• Perform admissions, recertifications, and discharges, with accurate and timely OASIS documentation
• Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition
• Educate patients and family caregivers on disease management, medications, and home safety
• Respond to changes in patient status and elevate appropriately
• Maintain documentation that meets Medicare, state, and agency compliance standards
