Assured Home Health in Redding/Chico, California, is hiring a full-time Home Health RN to provide and direct nursing care in patients’ homes as prescribed by physicians and compliant with laws and agency policies.

You will coordinate total care plans with healthcare professionals, patients, families, and community resources to ensure continuity of care. The role includes per-visit and hourly compensation structures and a comprehensive benefits package.

Formación

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsabilidades

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient’s care team from admission to discharge.
• Complete clinical nursing assessments per federal/state program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meet patient needs and obtain physician approval for revisions.
• Function across various home environments and with diverse patient populations.
