Job summary

Enhabit Home Health & Hospice is seeking a compassionate licensed professional to administer skilled care to patients in the home setting. You will teach patients and family members, collaborate with the health care team, and implement the plan of care as ordered by physicians.

Candidates must be licensed in their state, have at least one year of experience, CPR certification, and basic technology skills. A valid driver’s license and reliable transportation are required.

Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.
• Must be licensed in the state where they practice.
• Must have at least one year experience as a licensed professional.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
