Job summary

Trinity Health At Home is seeking a Home Health Registered Nurse (RN) to deliver skilled care in patients' homes in the Hartford region. You will develop individualized care plans, administer medications, and educate patients and families to promote recovery and independence.

This role offers schedule stability with Mon–Fri 8 am–5 pm, weekend rotation every 4 weeks, two holidays, a $10,000 sign-on bonus, and supportive nurse-led leadership.

Qualifications

• Graduate of an approved nursing program; BSN preferred.
• Licensed RN in the state of Connecticut.
• Minimum of 1 year experience as a professional acute care nurse.
• Home care experience preferred but not required.
• Must have a current driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Create individualized care plans and administer medications.
• Educate patients and families to support recovery and independence.
• Practice at the top of your license with 1:1 patient relationships.
