Job summary

Enhabit Home Health & Hospice is seeking a licensed healthcare professional to administer skilled care in the home. You will teach patients and families and work with the health care team, following physician orders and the established plan of care (POC).

Must have an approved school of clinical education, state license, and at least one year of licensable experience; basic tech skills including mobile-device use are required. A valid driver’s license and reliable transportation are essential.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
