Pathways Healthcare is a physician- and nurse practitioner-led home health and hospice provider delivering patient-centered care. This full-time RN role serves Cranston, Fall River and surrounding areas, with a focus on geriatrics and in-home treatment.

Responsibilities include initiating nursing procedures, administering medications, revising care plans, after-hours triage, and collaborating with physicians.

Qualifications

• Nursing degree from an accredited institution.
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilités

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
