Job summary

Compassus is seeking a registered nurse to plan, implement, and supervise home-based skilled nursing care in the Portland-North Coast area. You will collaborate with an interdisciplinary team including physicians, patients, and families to coordinate care in patients' homes.

The role requires an active RN license, a valid driver's license, CPR, and a minimum of two years of RN experience in a clinical or home health setting.

Qualifications

• Graduate of an accredited college/university with an RN nursing degree.
• Current, unencumbered State RN license.
• Current driver's license.
• Current CPR, TB test negative and hepatitis consent/declination.
• Two years of experience as a registered nurse in a clinical/home health setting.

Responsibilities

• Assesses home health patients to identify physical, psychosocial, and environmental needs.
• Case manages and provides clinical oversight to the interdisciplinary team, physician, and family.
• Implements, develops, documents the plan of care and revises as necessary.
• Provides care utilizing infection control measures that protect staff and patients (OSHA).
• Supervises and directs the HHA/LVN to ensure quality and continuity of services.
• Ensures continuity of quality patient care with appropriate documentation.
• Monitors cases to ensure compliance with third party payor requirements.
• Performs on-call responsibilities and provides after-hours care as assigned.
• Communicates effectively with patient/family, team members, physicians, and other health professionals.
• Adheres to all company policies and information security procedures.

Company: Providence at Home with Compassus. This position is part of a bargaining unit with a collective bargaining agreement.

Shift: Weekdays, day shift. Schedule: Full time. Work Location: Portland-North Coast OR Home Health.

Position Summary

Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assume the responsibility for the total provision of care.

Essential Duties and Responsibilities

• Assesses home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations
• Case manages and provides clinical oversight and direction to the interdisciplinary team, physician, and family
• Implements/develops/documents the plan of care and revises as necessary
• Provides care utilizing infection control measures that protect both the staff and the patient (OSHA)
• Supervises and provides directions to the HHA/LVN to ensure quality and continuity of services provided
• Assures continuity of quality patient care delivered with appropriate documentation
• Monitors assigned cases to ensure compliance with requirements of third party payors
• Performs on-call responsibilities and provides on-call service to patients/families, as assigned
• Provides effective communication to patient/family, team members, physicians, and other health care professionals
• Adheres to all company policies and procedures
• Adherence to and compliance with information systems security is everyone's responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered. Report information systems security problems.

Minimum Qualifications

• Education/Licensing/Certification: Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing
• Current, unencumbered State License as a registered Nurse
• Current Driver's License
• Current CPR, negative TB screen and Hepatitis consent/declination
• Experience: Two years of experience as a registered nurse in a clinical care setting, home health preferred.
• Knowledge and Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.

Compensation

The current compensation range for this position is $53.27 – $74.30 per hour. The specific rate of pay offered to a selected candidate will be determined based on relevant education, training, experience, geographic location, and other position-related factors. Compassus does not inquire about, consider, or rely upon an applicant's prior salary history in determining compensation. In addition to base compensation for the position, an employee working in this position may be eligible for pay differentials based on hours, shifts worked, or other criteria. They may also qualify for bonuses under applicable bonus programs or at Compassus's discretion, as well as other forms of variable pay under relevant company policies. A summary of applicable pay programs will be provided during the offer process.
