Job summary

Trinity Health Of New England At Home seeks a Home Care Registered Nurse to deliver primary nursing care in patients' homes, following physician orders and agency guidelines. You'll help develop individualized care plans and supervise aides as needed, aiming for excellent patient outcomes and satisfaction.

Responsibilities include admission assessments, therapy delivery, coordination with multiple disciplines, and timely documentation.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Connecticut.
• One year experience as a professional acute care nurse.
• Home health care experience preferred but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Plan and provide primary nursing care to home-based patients as ordered by the physician.
• Develop and update care plans in collaboration with physician and client.
• Provide therapy using approved techniques and modalities as needed.
• Coordinate care and referrals with other disciplines.
• Educate clients and caregivers on therapy programs and home exercises.
• Monitor progress and report changes to the physician promptly.
• Complete documentation at point of care in a timely manner.
