Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Woonsocket, RI area. The RN will provide skilled nursing and rehabilitative care in clients' homes, working with a multi-disciplinary team to ensure compassionate, high-quality care and to help clients remain at home.

Responsibilities include head-to-toe assessments, medication reconciliation, wound care, and real-time clinical decisions, with a focus on independent work and prioritizing urgent

Qualifications

• Active RN license in good standing in MA
• Graduate from an accredited nursing program
• At least 1 year of patient care experience with adult/geriatric populations in acute care, home health, hospice, or skilled nursing settings

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition
• Triage and manage urgent clinical situations in the home setting
• Perform medication reconciliation, patient teaching, and safety monitoring
• Conduct comprehensive wound assessment, care, and documentation
• Maintain accurate, real-time clinical documentation and adapt to dynamic visit schedules
