Alternate Solutions Health Network in Ohio is seeking a Registered Nurse (RN) to deliver skilled nursing care in patients’ homes across Dublin, Lewis Center, Plain City, and surrounding areas. You will work autonomously with an interdisciplinary team, coordinating care and educating families to support recovery and quality of life.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting.
• Home care experience preferred.

Responsibilities

• Plan of Care: Observe and monitor patient conditions and perform OASIS assessments to develop individualized care plans.
• Minimize Patient Risk: Administer medications as prescribed and prioritize visits for high-risk patients.
• Continuity of Care: Manage multi-disciplinary care and ensure appropriate admissions, transfers, and discharges.
• Family Educator/Advocate: Educate patient and family using teach-back methods.
• Policies: Complete clinical documentation following agency protocol and Medicare guidelines.
• Operations: Attend in-service training and agency meetings.
