Job summary

Guardian Angel Home Care is seeking a compassionate Registered Nurse (RN) to provide skilled nursing care in patients' homes within the assigned service area.

The Home Health RN performs assessments, develops care plans, and coordinates with physicians and therapists while ensuring compliance with CMS, CHAP, Medicare, and state regulations. This field-based role offers autonomy with strong team support and the potential for full-time, part-time, or per diem schedules.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• Valid driver's license and reliable transportation.
• Current CPR certification.

Responsibilities

• Perform comprehensive patient assessments including OASIS when required.
• Develop, implement, and update the Plan of Care with physicians and the interdisciplinary team.
• Provide skilled nursing treatments and interventions as ordered by the physician.
• Monitor patient progress and document outcomes of care.
• Coordinate services with therapists, aides, and other healthcare professionals.
• Ensure CMS, Medicare, CHAP, and agency standards compliance.
• Educate patients and families on disease management and safety.
• Supervise LPN/LVN staff and home health aides where appropriate.
