Job summary
Guardian Angel Home Care Inc. is seeking a Registered Nurse (RN) for home health field visits across the service area. The role focuses on delivering skilled nursing care to patients in their homes, coordinating with physicians and the interdisciplinary team.
The position offers flexible scheduling, autonomy in the field, and strong earning potential based on productivity and visit volume. Travel between patient homes within assigned territories is required, with comprehensive compliance to CMS,
Qualifications
- Active RN license in the state of employment.
- Graduate of an accredited School of Nursing.
- Valid driver's license and reliable transportation.
- Current CPR certification.
- Experience with OASIS documentation (preferred).
- Home health experience (preferred).
- Strong assessment and critical thinking skills (preferred).
Responsibilities
- Conduct comprehensive patient assessments including OASIS when required.
- Develop and update Plan of Care with physicians and team.
- Provide skilled nursing treatments as ordered by physician.
- Monitor progress and document outcomes of care.
- Communicate patient status to physicians and care team.
- Coordinate services with therapists, aides, and other professionals.
- Maintain compliant clinical documentation and participate in audits.
