Job summary

Compassus is seeking a Home Health Registered Nurse (RN) to support patient recovery at home and empower families to achieve healthcare goals through education and care planning. You will provide clinical oversight to an interdisciplinary team and coordinate care in the home setting.

Qualified candidates will hold an RN license in Michigan, have 2+ years of RN experience (home health preferred), and possess a current driver’s license with CPR/BCLS certification.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, home health experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Support patient recovery and ability to stay at home – enabling them to spend quality time with their loved ones and doing the activities they enjoy.
• Educate patients and families to reach healthcare goals by disease management.
• Case manage and provide clinical oversight to interdisciplinary team.
