Job summary

Addus HealthCare in Great Falls, Montana, hires Registered Nurse or LPN to support in-home patient care with a flexible schedule designed to meet client needs. The role emphasizes one-on-one client engagement, initial assessments, and developing individualized care plans.

You will conduct supervisory visits, monitor medication services, and collaborate with case managers to ensure exceptional client service and outcomes. Travel within the local area is required.

Qualifications

• Graduation from an accredited school of nursing and current LPN/RN licensure in Montana.
• Excellent assessment and communication skills.
• Must be able to travel locally in your own vehicle with valid driver’s license and insurance.

Responsibilities

• Enable patients to stay independent while they remain in their own homes.
• Develop a plan of care with clients and families during initial assessment.
• Perform supervisory visits to evaluate the plan of care and ensure high-quality service.
• Monitor and supervise medication services.
• Occasional delegation to non-licensed staff.
• Deliver educational in-service and initial trainings.
• Communicate with Case Managers and other care team members.
• Demonstrate clinical excellence and personal compassion.
• Promote our positive image in the community as the leading home care provider.
