Job summary

Qualifications

• Graduation from an accredited nursing program and Montana LPN/RN licensure.
• Excellent assessment and communication skills.
• Reliable, energetic, self-motivated and well-organized.
• Willingness to travel locally in own vehicle with a valid driver’s license and insurance.

Responsibilities

• Develop plan of care through initial assessment.
• Perform supervisory visits to evaluate the plan of care.
• Monitor and supervise medication services.
• Communicate with Case Managers and other members of the care team.
• Present educational in-service and initial trainings.
• Promote our positive image in the community as the leading home care provider.
