Addus HomeCare is seeking a dedicated Registered Nurse to provide skilled visits to patients across Sussex and Kent counties. You will assess conditions, coordinate care, and implement plans of care under the Clinical Services Manager supervision.

The role emphasizes independence, a flexible schedule, and strong teamwork with an emphasis on quality patient outcomes and adherence to company policies.

Qualifications

• RN must be licensed in the state.
• 1 year clinical nursing experience preferred (ICU/rehab/med-surg).
• Home health experience preferred.
• CPR certification must be current.

Responsibilities

• Provide visits as assigned by Clinical Services Manager, including assessment and planning.
• Coordinate care with interdisciplinary team and other agencies.
• Communicate with attending physician to optimize patient care.
• Update care plans based on patient needs and licensure.
• Perform nursing procedures as directed by physician.
• Maintain accurate patient records and timely care plans.
