Job summary

Addus HealthCare is seeking a professional Registered Nurse to deliver skilled nursing care to patients in Sussex and Kent counties, DE. The role emphasizes a flexible schedule and independent work with support from the Clinical Services Manager.

You will perform assessments, implement plans of care, coordinate with the interdisciplinary team, and document care provided. Ideal candidates have RN licensure in Delaware, at least 1 year of clinical experience (ICU/rehab/med-surg preferred), CPR

Qualifications

• Registered Nurse licensed in Delaware.
• At least 1 year clinical experience in ICU/rehab/med-surg preferred.
• Home health experience preferred.
• Valid CPR certification.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide visits as assigned by Clinical Services Manager, including assessment and care planning.
• Assess patient data from physical, psychological, social and spiritual factors to coordinate care.
• Communicate with attending physician and interdisciplinary team to coordinate optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform and document therapeutic, preventive and rehabilitative nursing procedures as directed.
• Pronounce death when applicable and support family.
• Maintain accurate patient records and timely care plans.

Full Time: Monday through Friday 8 am to 5 pm.

RN What You'll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification.
• Must have reliable transportation, current driver's license and appropriate automobile insurance
