Qualifications

• Must have RN license in the state where practicing.
• CPR certification required.
• Excellent communication with patients and families.
• Ability to travel to patients' homes.
• On-call responsibilities as assigned.

Responsibilities

• Promote mission, vision, and values.
• Complete initial and ongoing assessments (including OASIS) for home health clients.
• Reevaluate nursing needs and outcomes; assist in recovery and maintenance.
• Develop, initiate, and revise the Plan of Care; plan for discharge.
• Communicate with clients, families, and health professionals; participate in case conferences.
• Apply infection control and universal precautions.
• Coordinate changes with physicians and team; inform of condition changes.
• Monitor cases for third-party payer requirements.
• Complete timely documentation per policy.
• Supervise and provide clinical direction to other nursing staff.
• Perform on-call duties as assigned.
• Engage in in-service programs and maintain competency.
• Meet mandatory continuing education requirements.
