Job summary

River Valley, part of The Pennant Group, is seeking a Registered Nurse to coordinate and deliver home health services in a community setting. The role focuses on planning, coordinating care, patient education, and collaboration with physicians and caregivers.

The RN will lead interdisciplinary teams, ensure adherence to care plans, supervise home health aides, and support patients in their home environments. A valid RN license and CPR certification are required.

Qualifications

• Must have graduated from an approved school of nursing and be licensed in the state where practicing.
• Minimum of two years of experience, at least one in public health or home care nursing.
• Maintains a current CPR certification.
• Ability to drive and have transportation for agency travel; must have valid driver’s license and automobile insurance.
• Self-directed and able to work with minimal supervision.
• Demonstrates excellent observation, problem solving, verbal and written communication skills.
• Experience supervising home health aides.

Responsibilities

• Develops and initiates the plan of care and performs ongoing interdisciplinary assessment and development of the individualized plan of care with patient, family, and caregivers.
• Initiates appropriate nursing procedures and provides services ordered by the physician as indicated in the plan of care.
• Provides patient, caregiver, and family counseling and education.
• Prepares clinical notes and communicates with the physician responsible for the home health plan.
• Identifies discharge planning needs and implements prior to discharge.
• Keeps primary physician updated on patient status at least every sixty days.
