Job summary

UniversaCare is hiring Registered Nurses to serve the Connecticut Shoreline communities including New Haven area, Branford, Guilford, Madison, Clinton, Old Saybrook, Old Lyme and surrounding areas. Flexible scheduling is available, with opportunities to provide in-home care.

You will assess health, develop care plans, administer medications, monitor progress, educate families, and document care while collaborating with physicians and the care team.

Qualifications

• Active Connecticut RN license in good standing.
• Home health experience is a plus.
• Strong clinical assessment and communication skills.
• Reliable, compassionate, and patient-focused.
• Ability to work independently with a care-team collaboration.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Assess patients' health conditions and develop care plans.
• Administer medications and treatments as ordered.
• Monitor patient progress and communicate changes in condition.
• Collaborate with physicians, caregivers, families, and the healthcare team.
• Educate patients and families regarding care, medications, and health management.
• Maintain accurate and timely clinical documentation.
• Follow all applicable healthcare, safety, and infection-control standards.
• Provide compassionate, patient-centered care at every visit.
