Job summary

AdvisaCare is seeking a skilled Home Health Registered Nurse (RN) to serve the Greater Portage and Kalamazoo County areas. You will lead the care team, perform advanced clinical tasks, and provide vital education to patients and their caregivers.

We welcome RNs (and LPNs under RN supervision) with 1+ year home health experience. Flexible full-time or part-time scheduling and travel within a ~30 mile radius are offered to provide compassionate, personalized care.

Qualifications

• Michigan RN/LPN licensure required with Home Health experience preferred.
• Ability to provide one-on-one patient education and advocacy.
• Flexibility for full-time or part-time scheduling; travel within ~30 miles.

Responsibilities

• Lead the care team and deliver advanced clinical tasks in home health.
• Perform wound care, infusions, and related procedures; educate patients and caregivers.
• Coordinate care with a multidisciplinary team to achieve optimal outcomes.
