Job summary

Navicent Health Medical Center is seeking an RN for Home Health in Macon, GA. The role involves managing a patient caseload, performing in-home visits, and coordinating care with a multidisciplinary team. Schedule is Monday–Friday with occasional weeknights and weekends on call; 40 hours per week with a sign-on incentive up to $3,000.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• BLS certification required per policy.
• Current RN license in the state of work; Nurse Practitioner not required.

Responsibilities

• Manage daily patient caseload and document care per plan of care and MD orders.
• Collect health data from home assessments and EMR; communicate findings.
• Analyze data to identify issues and coordinate resources.
• Identify high-risk patients and coordinate interventions to reduce re-hospitalization.
• Coordinate care with a multidisciplinary team and supervise LPNs/HHAs.
• Perform treatments and procedures per state and agency policies.
• Evaluate progress toward patient-centered goals.
