Job summary

Concierge Home Care in Fort Walton Beach, FL is seeking a Home Health Registered Nurse dedicated to patient-centered care in the home setting. You will deliver skilled nursing, create care plans, and collaborate with physicians and care teams to support recovery and independence.

We offer flexible scheduling (PRN, full-time, modified), onboarding and mentorship, CEUs, health insurance after 30 days, and PTO from day one with a supportive leadership team and modern AI-assisted charting tools.

Qualifications

• Active, unencumbered Florida RN license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience preferred but not required.

Responsibilities

• Conduct patient assessments and develop care plans.
• Provide skilled nursing care and patient education.
• Coordinate care with physicians and interdisciplinary teams.
• Monitor patient progress and adjust care as needed.
• Document visits efficiently using AI-assisted tools.
