Job summary

BAYADA Home Health Care is seeking a full-time Registered Nurse to join our Vermont home health visit teams, providing skilled nursing and rehabilitative care in clients’ homes to help them stay healthy at home.

You’ll assess, plan, and coordinate with a multidisciplinary team, handle urgent visits, and document care with point-of-care technology. Competitive pay with a weekly schedule and bonus opportunities for full-time roles.

Qualifications

• Current Vermont RN license in good standing
• Graduation from an accredited nursing program
• At least one year of medical experience with adults/geriatric population in acute care, ED, home health, hospice, ICU, med-surg or rehab in past two years

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition
• Manage acute and post-acute care needs (CHF, COPD, wounds, IV therapy, post-surgical care)
• Triage and independently manage urgent clinical situations at home
• Perform medication reconciliation, teaching, and safety monitoring
• Document wound care and condition in the chart
• Work independently while escalating concerns appropriately
