Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Home Health Registered Nurse to join our team in Demorest, GA. This role focuses on 1:1 patient care in the home setting, with emphasis on compassion, independence, and collaborative teamwork to enhance patient outcomes.

You will assess patient needs, develop care plans, administer treatments, and provide education to families.

Qualifikationen

• RN license in the state of practice and in residence is required.
• Minimum 1 year nursing experience; home health/geriatrics preferred.
• Valid driver's license and proof of car insurance.
• New grads may be considered in select markets.
• Current CPR certification required.

Aufgaben

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
