Job summary

Elara Caring in Illinois is seeking a Registered Nurse Home Health to join our team as a PRN role. You’ll assess patients at home, plan and implement care, and collaborate with an interdisciplinary team to promote patient independence.

This role requires 1+ year of nursing experience, a current RN license, and willingness to travel up to 50% with a reliable vehicle. We offer a competitive base salary range of $45.00–$55.00 per hour, medical/dental/vision after 30 days, a 401(k) match, and paid

Qualifications

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Ability to travel up to 50% with a dependable vehicle.
• Associates or Bachelor’s Degree in Nursing is preferred.

Responsibilities

• Assesses assigned home health patients to identify physical, psychosocial, and environmental needs.
• Assumes primary responsibility for caseload, planning and evaluating the plan of care.
• Communicates significant findings to the Clinical Supervisor and care team; updates plan as needed.
• Maintains patient privacy and documentation in accordance with HIPAA.
