HomeCentris, a leading home health provider in Fairfax, VA, is seeking an experienced Registered Nurse to deliver skilled nursing care in clients’ homes. You will assess health status, develop care plans, administer treatments, and educate patients and families while coordinating with the broader health team.

Qualifications

• Current unrestricted RN licensure in the state.
• Minimum of two years of clinical RN experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently and manage a diverse caseload.

Responsibilities

• Assess patients and develop individualized care plans.
• Provide skilled nursing care including wound care and meds as prescribed.
• Educate patients and families on disease management and self-care.
• Coordinate with therapists, social workers and aides for continuity of care.
• Document nursing assessments and progress in EHR per agency policy.
• Administer medications and monitor effects per physician.
• Advocate for patients and families to address needs.
• Ensure safety and infection control in home environments.
