Job summary

Mercy Home Health is seeking an experienced RN to deliver comprehensive in-home patient care across the Mercy South area and neighboring counties. This role involves autonomous community-based nursing, patient education, and collaboration with a multi-disciplinary team.

Applicants should have an accredited nursing degree and an active Missouri RN license, with BLS certification. A valid driver’s license and auto insurance are required; mileage is reimbursed.

Qualifications

• Graduation from an accredited nursing program is required.
• One year of related home health or acute care experience preferred.
• Current RN license in Missouri is required.
• BLS Certification required through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in home setting including case management and total care for patients with varying acuity.
• Perform admissions, post-hospitalization assessments, recertifications, discharges, and patient teaching.
• Document care in Epic and maintain timely, accurate records.
