Qualifications

• 2 years experience as a Registered Nurse, home health experience preferred.
• Registered Nurse with Valid (or eligible) Connecticut license.
• Ability to perform tasks involving physical activity, including heavy lifting.

Responsibilities

• Assess/reassess patients and establish patient-specific care plans.
• Collaborate with providers and communicate changes promptly.
• Educate and instruct patients/caregivers on medical maintenance.
