The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team. You will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services.

Responsibilities include assessing, planning, implementing and evaluating care, documenting plans of care, and revising the plan as needed. Requirements: Florida RN license, Health Certificate, PPD/Chest X-Ray, BLS, and at least one year of RN experience.

Qualifications

• Current Florida RN license
• Current Health Certificate
• PPD or Chest X-Ray
• BLS certification
• One year RN experience preferred

Responsabilités

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Document the patient’s plan of care using nursing diagnoses, outcomes, and interventions.
• Perform interventions according to priorities, plan of care and hospital/policy standards.
• Revise the plan of care as patient needs change and as evaluation indicates.
