KeystoneCare Home Health and Hospice is seeking a dedicated Registered Nurse (RN) to join our Home Health team on a full-time basis. As a Home Health RN, you will assess patient needs, develop personalized care plans, and provide hands-on nursing care in patients' homes, collaborating with physicians and the interdisciplinary team to ensure quality, patient-centered care.

Qualifications

• Current RN license in good standing to practice in Pennsylvania.
• BSN preferred.
• At least 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and valid Driver's License.
• Proof of COVID-19 vaccination or approved exemptions.
• Proof of annual influenza vaccination or approved exemptions.

Responsabilités

• Conduct comprehensive patient assessments to evaluate healthcare needs.
• Develop, implement, and update individualized care plans based on patient assessments and preferences.
• Administer medications and treatments as prescribed by the physician.
• Monitor and document patient progress and responses to treatment.
• Educate patients and families on health management, medication administration, and care plans.
• Coordinate care with interdisciplinary team members to promote optimal health outcomes.
• Ensure compliance with agency policies, state, and federal regulations.
• Act as a patient advocate, promoting rights and preferences in care.
