KeystoneCare seeks a Registered Nurse – Home Health to deliver high‑quality nursing care in patients' homes across Montgomery County and surrounding areas. You’ll assess needs, develop tailored care plans, and coordinate with the healthcare team to support independence.

This role emphasizes compassionate patient education, strong clinical judgment, and reliable transportation to meet patients where they live.

Qualifikationen

• Must be a graduate of an accredited nursing program.
• Current PA RN license to practice.
• Experience in home health or community health preferred.
• Strong clinical assessment and critical thinking.
• Ability to communicate effectively with patients, families, and team.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Understanding of home health regulations and procedures.
• Proficient in utilizing electronic health records.
• Annual influenza vaccination or an approved exemption.

Aufgaben

• Conduct comprehensive assessments of patients’ health and functional status to determine eligibility for home health services.
• Collaborate with physicians to develop and implement individualized care plans.
• Educate patients and their families about managing illnesses and promoting health.
• Monitor patients’ progress and adjust care plans as necessary based on clinical judgment.
• Document all patient interactions, assessments, and care provided in the EMR.
• Coordinate with interdisciplinary team members to ensure comprehensive care delivery.
• Provide referrals for additional services as needed to support patient care.
• Adhere to infection control protocols and maintain a safe care environment.
