Job summary

Phoenix Home Care is seeking a registered nurse or licensed practical nurse to provide skilled home health visits in Moberly, MO and surrounding areas. You will assess patients, implement physician orders, and coordinate with therapists to form individualized care plans.

Expect collaborative teams, flexible scheduling, and the chance to impact patients’ quality of life. Responsibilities include charting during visits, updating plans of care, and ongoing patient education.

Qualifications

• Must hold MO RN or LPN license.
• Home Health or Critical Care preferred.
• Valid driver’s license with vehicle and auto insurance.
• Ability to pass drug test, background check, and physical.

Responsibilities

• Assists in development, review and revision of the plan of care.
• Provides nursing care per physician’s plan and client’s plan of care.
• Observes, records, and reports reactions to treatment and changes in condition.
• Provides ongoing instruction and supervision of client, family, or other health team members in the plan of treatment.
• Participates in education programs, quality assessment activities, nurse meetings, and other staff meetings.
• Monitors and records patient condition and documents care via electronic charting.
