Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team, applying the nursing process and delivering patient care under the direction of the Director of Clinical Services.

Responsibilities include assessing, planning, implementing, and evaluating care, documenting plans, and adapting the plan of care as needed while maintaining high clinical standards. Competitive pay and flexible scheduling are offered to support quality home-based care.

Qualifications

• Current RN license for the state where you practice.
• Current Health Certificate (per facility RN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collects, analyzes, and interprets data from health care members and documents diagnoses.
• Documents the plan of care using identified nursing diagnoses, expected outcomes, and interventions.
• Performs interventions according to priorities, plan of care and hospital policies.
• Revises the plan of care according to evaluation and changes in the medical plan of care.
• Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor.
• Other RN duties as assigned.
