Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team. The RN will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services, demonstrating clinical judgment in a home care setting.

Responsibilities include assessing, planning, implementing, and evaluating care, documenting plans, and adjusting care as needed. Competitive salary with flexible work hours and patient-centric scheduling are offered.

Qualifications

• Current Registered Nurse (RN) license for the state in which the nurse practices.
• Current Health Certificate (per facility RN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data from health care members and documents nursing diagnoses.
• Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
• Performs interventions according to identified priorities, plan of care, and hospital policies.
• Revises the plan of care according to evaluation and changes in medical plan of care.
• Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor.
• Other Registered Nurse (RN) duties as assigned.
