Advantage Home Health Services in Carlisle/Mechanicsburg, PA is seeking a full-time Home Health Registered Nurse to deliver compassionate patient care in home settings.

Qualifications

• Active RN license in the state of practice or New Grad with eligibility to take NCLEX.
• Experience in home health is a plus.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS timely.
• Draft care plans and reconcile patient medications.
• Prepare accurate, timely documentation.
• Coordinate care with patient and provide reports to Case Manager.
• Provide health care instructions to patient/family as appropriate.
