Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Develops, implements, and updates patient care plans with physicians and the team; ensures home-health eligibility and conducts assessments.
• Provides and coordinates high-quality clinical care; supervises home health aides and LPN/LVNs per regulations.
• Documents all patient care per policy and payer requirements; communicates with physicians, patients, families, and staff.
• Participates in orientation and precepting of new staff; contributes to performance improvement and patient safety initiatives.
• Demonstrates professionalism through punctuality, ongoing learning, and adherence to standards; participates in on-call/weekend rotations.
• Function effectively in any home setting regardless of demographic factors.
