Gunnison Valley Health in Gunnison, CO is hiring a Home Health Nurse to join a small, specialized home care team. You will provide skilled nursing services to patients in their homes, perform initial evaluations, and coordinate care with physicians and family members.

Qualifications

• Must possess a Nursing Degree and a current RN license in Colorado.
• At least 1 year of experience in a hospital, skilled nursing facility, or similar.
• Wound care experience is a plus.

Responsibilities

• Administer skilled nursing care to patients requiring intermittent professional nursing services.
• Complete initial evaluation visit and re-evaluate nursing needs.
• Coordinate services with physicians and other personnel; counsel patients and families.
• Participate in in-service programs and supervise/train other nursing personnel.
