Qualifications

• Active RN license and CPR certification required.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health/Med-Surg/ICU preferred).

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medications and interventions.
• Coordinate care across therapists, aides, and physicians as case manager.
• Monitor patient progress and adjust care based on changing conditions and education.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides and provide support.
• Participate in on-call rotation as appropriate.
